Glossary of selected medical terms

By creating this section, I would like to introduce you to popular terms directly related to the profile of my business. Of course, the explanations contained therein do not replace a medical consultation, nor will they reflect the fullness of medical knowledge, However, I have hope, that will allow you to gain insight into the topics that interest you. I will try to systematically supplement the Glossary with new terms, and I will be grateful for any suggestions.

  • Laser ablation

laser ablation, laser removal, burningA term most often referring to the process of removing lesions from the skin surface, consisting in transforming them into a gaseous state, bypassing the liquid state. Short- and ultra-short-pulse lasers are usually used to induce ablation. The process of tissue evaporation can also occur using a continuous laser, as long as the radiation beam is of sufficiently high power.

  • Anoderma

anoderma, salesA type of skin lining the final section of the anal canal (leather belt, anatomical channel) peripherally from the crest line (gear line), gradually passing into the anal verge, and then into the normal skin of the buttocks. It is covered with non-keratinized stratified squamous epithelium and devoid of skin appendages. The anal verge is a strip of skin with a diameter of approx. 5 cm, extending around the anus. It has a different structure - it is covered with keratinized stratified squamous epithelium containing pigment, hair follicles and sweat and sebaceous glands, called perianal glands, which secrete a special secretion, different from the secretion of ordinary skin glands. The anoderm has rich sensory and pain innervation, therefore, diseases developing within it are usually accompanied by severe pain. The most common diseases in this area are condyloma acuminata, anal fissure and marginal thrombus.

  • anoscopy

anoscopy, rectal examinationType of proctological examination, allowing the assessment of the anal canal and the final section of the rectum. The examination involves inserting a short speculum (anoscope) through the anus and examining this area. It is also possible to simultaneously perform simple therapeutic procedures and take samples for histological examination. Anoscopy takes no longer than a few minutes, is completely safe, and complications are rare and most often consist of minor bleeding.

  • Anticoagulants; information for patients

acenocoumarol, anticoagulantsIn health conditions, the activity of prothrombin group proteins, i.e. blood clotting factors, ranges from 75 do 110% (1.5 – 0.9 INR). In case of thrombotic disease, pulmonary embolism, atrial fibrillation, after implantation of an artificial heart valve, artificial vascular prosthesis, etc. this amount is too large and promotes the formation of clots and embolisms. The drugs Acenocumarol and Warfarin inhibit the formation of clotting factors in the liver and thus reduce the activity of prothrombin.. Decreased clotting (prothrombin activity) to the so-called. therapeutic width range, i.e. to the value of the index from 30 do 50% (4.5 – 2.4 INR), is conduct, which significantly reduces the risk of clots and facilitates the withdrawal of existing ones. It is most beneficial to reduce the prothrombin activity slightly below 50% (2.4 INR). Excessive reduction of blood clotting is dangerous and may result in bleeding complications (nosebleed, bleeding gums, hematuria or subcutaneous or intra-articular hemorrhages).

Therefore, antithrombotic treatment requires full cooperation between the patient and the doctor. Both strict dosing of the drug and timely reporting for analytical blood control 1 – 4 weeks must be strictly observed for the safe conduct of this treatment.

Anticoagulants should be taken once a day after breakfast at the dose determined by the doctor. The dose of the drug is determined by the doctor based on the results of the blood analysis. You should report for a blood test on the scheduled day in the morning. The subject does not have to fast.

Any patient taking Acenocumarol or Warfarin is guilty of this:

      • Always carry your anticoagulation card with you and show it at every medical examination.
      • Remember, that the use of other drugs must be strictly agreed upon either with the doctor conducting anticoagulant treatment, or you must show the card to the doctor giving you advice. Such agreement is necessary because some medications, such as. salicylaty (Aspirin, Calcipiryna, Asprocol), Butapirazol, Chinidyna, anti-tuberculosis drugs (NOT, Etionamid), oral antidiabetic drugs (Diabetol), sulfonamidy (Biseptol), antiatherosclerotic drugs, certain antibiotics (Oxyterracyna) and others have a blood clotting-lowering effect. Therefore, if the anticoagulant effect of the drug used is superimposed on the anticoagulant effect of other drugs, there is a risk of dangerous bleeding complications (as a result of decreased coagulation below the index value 30% (4.5 INR)). On the other hand, vitamins, barbiturany (Luminal), Antacids reduce the anticoagulant effect of anticoagulants, which may eliminate the effect of their action and lead to thrombotic complications – embolisms.
      • While taking Acenocumarol or Warfarin, the number of intramuscular injections should be limited to what is absolutely necessary.. If a tooth needs to be removed, possibly another treatment, you should consult with your doctor in advance to reduce the dosage of the drug accordingly, so that on the day of extraction (treatment) prothrombin activity was higher than 75% (1.5 INR).
      • During menstruation, the established dose should be reduced by half, and after the end of the menstrual period, return to the original dosage.
      • If bleeding complications occur, see a doctor immediately.
    • If the prothrombin index is found to be significantly different from the previous one, with proper treatment, its determination should be repeated immediately to rule out a laboratory error.
  • Keloid

bliznowiec, keloid, scarA keloid is a tumor that looks like an extremely hypertrophic scar, made of collagen fibers, were caused by excessive activity of fibroblasts. It most often appears at the site of injury, but also without any tangible cause. Keloids are usually seen around the neck and chest, but they can arise anywhere in the body. The tendency to develop keloid scars is genetically determined.

Removing keloids is extremely difficult and often ends in failure. Surgical removal of the lesion is itself an injury, and therefore a factor causing an undesirable reaction and may lead to the formation of another hypertrophy in the same place.

Various types of ointments and dressings are used to treat and prevent recurrence, however, they do not give satisfactory results. At the current level of knowledge, the most rational solution is to excise the keloid and then apply early treatment, steroid combination therapy in the form of local injection and application of ointments. However, this does not guarantee the absence of recurrence.

  • Seborrheic wart

seborrheic wartAlso called senile wart because it is proportionally more common in older people, is gentle, epidermal hyperplastic lesion of unclear etiology. Most often, it takes the form of multiple flat growths, convex or pedunculated, with sizes ranging from several to several mm, ranging in color from flesh-colored through brown to black and rough, granulated surface. Typically, seborrheic warts do not cause any discomfort, they are extremely rarely subject to malignant transformation. They are often subject to mechanical irritation, tear, crumbling. Large warts are surgically removed, smaller with a CO2 laser or the older electroresection method.

  • Viral warts

common warts, wartsAlso commonly called warts, warts are a local skin infection with the human papillomavirus (HPV). Infection occurs through direct or indirect contact with an infected person, and is favored by micro-injuries and softening of the skin. The most common characters are:

      • common warts – located in most cases on the fingers and nail folds, however, they can occur anywhere in the body. They take the form of grayish, rough bumps, calloused surface, often with tiny black dots, being clotted capillaries.
      • plantar warts – usually wider and flatter, sometimes merging into a mosaic form. In the initial phase of development, they are often confused with corns.
      • flat nipples – as the name suggests, superficial, small, flat, greyish or brownish nodules, typically located on the face and back of the hands.
    • genital warts. They are the most common venereal disease (herpes). It has also been proven that HPV infection is associated with penile cancer, vulva, uterus and rectum. The changes are located on the genitals, in the vagina, on the cervix, in the rectum. They often take the form of condyloma acuminata.

So far, no causal treatment methods for warts have been developed, and due to the aggressiveness of the virus, there is a high recurrence of changes. The only proven way is to remove them mechanically. Of the many known techniques, the most effective and safest for the patient is the use of a surgical CO laser for this purpose2.

  • Melanoma (mean)

malignant melanoma, rak, melanomaMelanoma is one of the cancers, among which the highest increase in incidence has recently been recorded. It is a malignant skin tumor originating from melanocytes - pigment cells that produce melanin. W 90% cases, it develops in the skin, but it can also arise in parts of the eyeball, nail bed, mucosa of the digestive tract and genitals. The starting point is pigmented moles (moles), although cases of the disease developing in unchanged skin have been described. Melanoma is characterized by high malignancy - it gives distant metastases to the lungs quite early, brain, and at the same time it is extremely resistant to the treatment used.

There are several characteristics of the birthmark, which should increase our oncological vigilance, because their presence raises the suspicion of a malignant lesion:

    • size above 5 mm
    • many colors from white through brown, pink, blue to black
    • irregular edges, asymmetry
    • irregular surface or non-healing ulcer
    • itching or pain
    • the appearance of an inflamed areola and any changes in appearance.

In the differentiation of melanoma, the first thing to consider is:: pigmented birthmark, vascular granuloma, seborrheic wart, dural fibroma, basal cell carcinoma.

The only appropriate therapeutic procedure is to remove the suspicious lesion as early as possible along with a margin of healthy skin and histopathologically examine the preparation.. Only early treatment allows for complete recovery.

Currently, intensive work is being carried out around the world on drugs used to fight melanoma, and in recent years there has been great progress in this field.. These include:. monoclonal antibodies, gene therapies and vaccines. In the case of vaccines, Poland is one of the leaders.

  • Boil

boilIt is a purulent perifollicular inflammation, most often caused by Staphylococcus aureus, characterized by the formation of a necrotic plug. It usually occurs on the face, neck, chest, buttocks. Initially, an inflammatory nodule is observed, and then the formation of a pimple on its top. In most cases, healing occurs on its own by forming an ulcer at the top of the pustule and evacuating the necrotic masses.. Sometimes surgical intervention is required, consisting in cutting it. Attempts to squish out a boil on your own carry the risk of generalizing the infection, including dangerous meningitis and encephalitis. Changes are usually single, but they can also occur in large numbers, especially in people suffering from additional diseases (Immune impairment, diabetes, cancer, kidney disease). Then we talk about furunculosis. Occurrence of multiple adjacent boils, that merge into one larger lesion is called a carbuncle (czyraka gromadnego). We most often find it on the back of the neck.

  • Defecation

defekacja, defecationDefecation (defecation) – złożony proces, which begins with a stimulus in the form of stretching of the walls of the rectum by the accumulated fecal masses, and ends with the closure of the anal canal after defecation. The brain is involved in the control of defecation, spinal cord and nerve centers in the large intestine. A healthy person should have a bowel movement no less than once every two days and no more than twice a day. Therefore, we distinguish stool passing disorders such as constipation (zatwardzenie, obstruction) and diarrhea (diarrhea).

  • Dermatoscopy

dermatoscopy, dermoscopyIt is a diagnostic method for assessing skin lesions by viewing them using a dermatoscope or videodermatoscope. Enlarged 10, and even 100 times estimated: symmetry, edge, color, diameter, evolution of change. Additionally, some small structures of the mole can be seen under the dermatoscope: dye mesh, larger balls and small pigment grains, dark brown streaks, blood vessels, places where the mark disappears. Each feature is scored accordingly. The sum of points indicates the need to remove the change or the possibility of leaving it. Of course, dermatoscopy is not a conclusive method and assesses the change only at the time of examination.

  • Disulfiram

disulfiram, wait, insertDisulfiram (Anticol, Antabuse, wait, pop. “insert”) is a means of helping people break the addiction of alcoholism by preventing them from drinking alcohol. During addiction treatment, it is used orally or in the form of tablets implanted under the skin, most often in one of the large muscles.. Professional psychotherapy should be conducted in parallel with pharmacological treatment. In therapeutic doses, it is a non-toxic compound, although sometimes side effects occur, np. weakness, metallic taste in the mouth, impotence.

The essence of action Disulfiram consists in inhibiting the enzymatic process responsible for the decomposition of ethyl alcohol and the accumulation of its metabolites in the body.. The effect of the implant lasts up to a year. After drinking a drink containing alcohol, skin redness appears, feeling of fear, dyspnoea, you experience nausea, vomiting, weakness, severe headaches, chest pain, fainting. Sometimes these symptoms slowly disappear and after a few hours of sleep the patient returns to normal. Alcohol consumption may also cause arrhythmias and heart attacks, convulsions, circulatory failure, in extreme cases, it can cause severe poisoning and even death.

Drug interactions Disulfiram: increases the toxic effects of alcohol (Attention: some syrups are made with alcohol), enhances the effect of anticoagulants, in combination with Metronidazole may cause acute psychosis.

Disulfiram is contraindicated in people with cardiovascular diseases, liver failure, hypothyroidism, epilepsy, as well as in people with suicidal thoughts. Particular caution should be exercised in patients treated with antihypertensive drugs. It should not be used earlier than 24 hours after the last alcohol consumption.

  • Electrosurgery

electrosurgery, electrocoagulationElectrosurgery is one of the older methods used in modern surgery. It involves cutting (electrotomii) and coagulation (electrocautery) tissues by diathermy using electric current. For this purpose, we use electrosurgical devices with a power of 40-300W. We use it to remove and destroy tissue with the possibility of cauterizing blood vessels at the same time. The undoubted advantages of electrosurgery are the low cost of the procedure and time savings. The disadvantages include low precision of the procedure and greater damage to surrounding tissues than in the case of newer methods (laser).

  • Ganglion

ganglion-2Ganglion (gelatinous cyst, mucous cyst) it's basically a pseudocyst – connective tissue diverticulum of the tendon sheath wall, filled with gelatinous content. It manifests itself as a small, tense and elastic bulge around the joint. It may cause pain by pressing on surrounding structures. The most common location is the wrist, less often the knee or foot. The cause is assumed to be inflammation of the tendon sheath, caused by chronic injuries (excessive exploitation). A change in the size of the nodule is often observed, depending on the load intensity of the nearby joint. Treatment often begins with puncture and aspiration of the contents, sometimes with an injection of a steroid drug. However, such treatment is rarely effective and the lesion recurs. Radical removal of the ganglion involves surgical excision.

  • Sweat adenoma

sweat adenomaIt takes the form of plural, slightly raised above the skin surface, small lumps with a smooth surface, slightly different in color from healthy skin.

Jak sugeruje nazwa, they have the shape of lumps ranging from a fraction to several mm in size and pearly in color. Pearly papules (pearly) they do not cause any discomfort and do not pose a threat to health, they only make it difficult to maintain penile hygiene. In diagnosis, they require differentiation from condyloma acuminata (genital warts). Basically, there are no absolute medical indications for their treatment, but most men want to get rid of lumps for aesthetic reasons. The most effective method is laser therapy using a CO2 laser. It is performed under local anesthesia and the procedure lasts several minutes. The remaining wounds after their removal heal in the draft 10 days, leaving the skin smooth. The procedure does not involve any complications and does not cause any dysfunction of the sexual organs.

  • Rubber banding of hemorrhoids (Barrona method)

rubber bandingThis is a popular invasive procedure, used to remove second and third degree rectal hemorrhoids. Using an anoscope, the enlarged lump is visible. It is then pulled into the gator cylinder using forceps (in my McGivney practice), and then slips off the garters (rubber rings) on the varicose vein stalk. The tightened ligature cuts off the blood supply and causes ischemia of the peripheral part of the hemorrhoid, which leads to its necrosis. As a result, the lump dies and falls off within 7-10 days. During one treatment, it is theoretically possible to put rubber bands on several hemorrhoids, however, treatments are usually performed one nodule at a time. This reduces the risk of complications after the ligation and possible post-treatment ailments. Subsequent hemorrhoids are removed at intervals of several weeks.

Barron's method gives very good results and is safe. After the procedure, you can immediately return to normal activity. Because rubber banding is done in the area a few centimeters above the ridge line, where there are no more pain endings, Usually, the only post-treatment symptoms are a feeling of fullness in the anal canal until the necrotic lump falls off. Some patients may experience minor pain for a few days.

  • Hemorrhoids

hemoroidy, hemorrhoids, hemoron, varicose veins of the anusThis is a colloquial synonym for the disease also known as hemorrhoids (loosestrife) or popularly, but incorrectly, as hemorrhoids. This is what we call the hypertrophied venous balls of the rectal plexuses, which, together with the symptoms they cause, create a picture of hemorrhoidal disease. Currently, it is treated as one of the diseases of civilization, as many of the famous ones, The factors causing it are related to the lifestyle of modern man. There are internal hemorrhoids, originating from the upper one, and external nodules from the lower rectal plexus. The border between them is the so-called. crest linea (zigzag line), i.e. the edge of the anal skin, where the strongly innervated anoderm turns into an insensitive mucosa.

The most common complaints reported by patients are burning sensations, itching, pain, feeling of incomplete evacuation and urge to defecate, bleeding, staining underwear with mucus and stools. Depending on the stage of the disease (I-IV degree) the lumps may not be visible or may fall outside the anus. Initially, hemorrhoids are usually treated symptomatically – conservatively . With severe symptoms, When the changes are significantly advanced or before a planned pregnancy, surgical treatment is recommended (Milligan-Morgana method) or minimally invasive using ligatures (rubber banding – Barrona method), laser, radio waves or the latest Hemoron method.

  • Hemoron

hemoron, hemorrhoid removal, treatment of hemorrhoidsThis method is the least invasive way to treat hemorrhoidal disease on an outpatient basis. The procedure is painless, lasts approx 30 min, and the patient is fully functional immediately afterwards. Treatment with this method allows for the removal of hemorrhoids, responsible for the tightness of the anus, and only restore their original size and functionality. This condition is achieved by treating the nodules with a device HEMORON, based on unipolar, low voltage diathermy.

The HEMORON method is completely safe for the patient. Thanks to it, it is possible to remove changes occurring in the large intestine, both in the initial one, first, as well as in the fourth, the most advanced, disease stage. The full course of treatment consists of: 90% patients received three treatments, performed at intervals – no more frequently than every three weeks and no less frequently than every five weeks.

Treatment with a non-invasive method HEMORON has been used for a year 2000, and all data indicate its high effectiveness. This technique was developed by American specialists under the name ULTROID and recognized as one of the standards of conduct. As shown by clinical studies, the procedure does not lead to visible scarring or purulent changes at the treatment site. The risk of possible complications is low. The patient can immediately return to his normal lifestyle.

  • Kaszak

kaszakIt is a stasis cyst of the sebaceous gland. It is located subcutaneously and has a soft form, a spherical tumor up to several centimeters in diameter. It is filled with stinks, porridge-like content, hence its name. If left untreated, the cyst slowly enlarges, it may fester and become punctured, resulting in scar tissue and usually recurrence. The treatment is exclusively surgical and involves surgical removal of the lesion. After complete excision, no recurrences are observed.

  • Yellow eyelid tufts

yellow tufts, xanthomasYellow tufts, or xanthomas, are single or multiple (jaundice) spilled, lumpy cholesterol deposits.

There are internal and external rectal plexuses. The border between them is the so-called. crested linea (comb line) where the strongly innervated anoderm turns into an insensitive mucosa. Their function is to seal the anus, which makes it easier to control the passing of gases and stools.

In disease conditions, when they are overfilled, i.e. congestion, normal balls enlarge, they connect with each other and undergo hypertrophy. They are then called hemorrhoids (hemorrhoids, loosestrife). Depending on it, which venous plexus is congested, internal hemorrhoids are distinguished, invisible in the initial phase of the disease and external, visible in the skin near the anus.

  • Condyloma acuminata

genital warts of the penis, vulva, anus, laser removal, wartsThe name "condyloma acuminata" comes from the macroscopic appearance of this form of genital warts, the most common venereal disease caused by the human papillomavirus (HPV). They take the form of elongated ones, cauliflower-like nodules, pink or not too different in color from the skin. They grow quickly and their development is favored by heat and moisture. The location of the changes is obviously related to the etiology of the disease - infection through direct sexual contact with an infected partner, although this may also occur through indirect contact. In women, condylomas usually occur in the perineum area, on the labia, in the vagina, on the cervix, in the rectum. In men, they are located on the inside of the foreskin, at the frenulum, on the glans, at the opening or inside the urethra, on the shaft of the penis, in the rectum and perineum. Warts may also appear in rarer locations – on the lips of the mouth, language, throat mucosa, larynx. Patients with advanced disease often consult a surgeon because they confuse condylomas with pearly penile papules., independent attempts at treatment or lack of proper medical diagnosis. If left untreated, they cause itching and burning, they block the urethra, they spread, infiltrating deeper tissues; they are also precancerous. Treatment is necessary, although very difficult; HPV is highly aggressive, there are no specific drugs to fight the virus. The best results are achieved by removing warts with a CO2 laser under infiltration anesthesia, although even here there is a high recurrence of changes.

  • Crossectomy

crossectomy, treatment of varicose veins, vein removalThis is a surgical procedure, consisting in cutting the largest vein of the superficial system of the lower limb – great saphenous vein and ligating its stumps just below the junction with the femoral vein. Its purpose is to eliminate reverse blood flow through the ineffective ostium – ostial valve – intersection with the deep vein system.

As an isolated procedure, crossectomy can be performed on an outpatient basis under local anesthesia (infiltration). The indications for the procedure are:: varicose vein disease when there are general health contraindications to classic surgery, lack of patient consent to hospitalization, confirmed reflux through the ostial valve with unchanged saphenous vein, as the first stage of invasive treatment for non-healing ulcers.

It is also an introduction to the removal of the great saphenous vein during surgery to remove varicose veins of the lower limb. During crossectomy, we tie off all tributaries around the mouth at the same time. Leaving them alone may result in the recurrence of varicose veins.

  • Bleeding and hemorrhage

bleedingThe difference in the definition of bleeding and hemorrhage is the rate and amount of blood lost. Bleeding is the slow flow of blood beyond the lumen of a damaged blood vessel, which rarely threatens the patient's life. Haemorrhage, it's heavy bleeding, rapid loss of whole blood from the body, which may cause hemorrhagic shock or death.

Bleeding is divided into internal bleeding, that is, the flow of blood into the body cavities, organs or tissues, direct external bleeding, i.e. the flow of blood outside the body and indirect, i.e. to spaces with direct communication with the surroundings.

  • Molluscum contagiosum

molluscumIt is an infectious disease, caused by a virus from the smallpox group. They appear hard on the skin, round, waxy white, shiny, semi-transparent nodules. Their characteristic feature is an umbilical depression in the central part, noticeable especially in larger changes. If we puncture and press such a lump, pearl-colored, porridge-like content comes out of it. Molluscum contagiosum nodules are numerous and can appear anywhere on the skin, and also on mucous membranes, although we most often find them on our hands, face and genitals. Lesions are rarely itchy or painful. The source of infection is other patients or shared objects. Infection also often occurs in swimming pools. The disease develops from several weeks to several months after contact with the virus. Lumps form in the infected location. The disease most often affects children and often heals on its own without leaving any scars. In other cases, dermatological treatment or surgical removal of lesions using cryotherapy or laser is necessary.

  • Miniphlebectomy

miniphlebectomyThis is a removal procedure (dissections, ligations and excisions) varicose veins of the lower limbs with micro-incisions (cuts from 2 do 3 mm). Special surgical tools are used for this purpose. Miniphlebectomy is a minimally invasive procedure, i.e. causing minimal trauma, minor discomfort and good cosmetic effect. The incisions are so small, that they are closed with a special plaster, single, with a thin seam or do not require any supplies at all.

This type of procedure can be performed on an outpatient basis if small ones are present, isolated varicose veins or as one of the elements of surgery for varicose veins of the lower limb with stripping of the great saphenous vein or saphenous vein. The outpatient procedure is performed under local anesthesia, and the patient does not have to interrupt normal life activities.

  • Calluses

laserCalluses, just like corns, are local hyperkeratosis of the skin, being a reaction to chronic pressure. Unlike them, they are flat, they do not have a horny core, they have a yellowish tint and blurred borders. They are most often located on the soles of the feet and palms. They usually do not cause any major discomfort. The reasons for their formation are similar to those of corns. We use exfoliating agents or mechanical shearing to remove calluses. In extreme cases, laser therapy may be useful. For treatment to be effective, it should also include removing the cause of the problem.

  • Hemangiomas

Hemangiomas are congenital or acquired hyperplastic lesions, classified as benign tumors. A rare malignant form is angiosarcoma. Hemangiomas arise from blood vessels (most cases) or lymphatic. We can find them practically everywhere – in the skin, mucous membranes, tires, brain, muscles, body organs, bones. However, they most often occur in women on the scalp and neck, where they take the capillary form (Structurally, they consist of a dense network of small vessels) or cavernous. Some congenital hemangiomas disappear spontaneously in the first years of life. The remaining ones can be treated surgically or minimally using a Nd laser:Yag.

  • Senile hemangiomas

hemangiomaSenile hemangiomas (rubiniaki, red moles, ruby spots or points, Cherry hemangiomas, Campbell de Morgan spots) are formed as a result of the proliferation of small blood vessels. They appear as spots or lumps of a bright red color several millimeters long. They are located on the skin in any area, but most often on the neckline and arms. Their frequency increases with age. They do not pose a disease problem and we remove them for cosmetic reasons, I guess, if they are caught and bleeding - then there are medical indications for their removal. We use a CO2 laser for the treatment, allowing one time, remove dozens of lesions without anesthesia. The resulting scabs fall off after a few days, usually leaving no trace.

  • Nagniotki

imprintNagniotki (prints) are foci of local hyperkeratosis of the skin, a reaction to chronic pressure. They are usually cone-shaped with a horny core, directed with its apex into the skin, which may cause pain. They are most often located on the soles and toes of the feet. The reason for their formation may lie in the irritating side (inappropriate footwear) or teased (degenerative changes in the feet with their deformation). To remove corn, we use softening and exfoliating agents. If this does not bring a satisfactory effect, surgery is necessary (cut, electroresection, laser). Treatment should also include removing the cause of the problem. The differential diagnosis should primarily include plantar warts, which are often confused with corns.

  • Obliteration

varicose veinsZamknięcie światła naczynia krwionośnego lub jamy ciała wskutek procesu chorobowego (przewlekłe procesy zapalne, zakrzepy, urazy) lub celowego działania lekarskiego (podanie leku obliterującego, thermal reaction). As a result, these spaces usually become overgrown with connective tissue, which is an irreversible effect.

Therapeutic obliteration is mainly used to remove varicose veins of the lower limbs and to close small vessels.

  • Sales

This is the end of the rectum, i.e. the final part of the large intestine. It is composed mainly of circular muscles (internal and external anal sphincter) surrounding the so-called. anal canal. During defecation through the anus, feces are expelled from the digestive tract. The most common diseases of the anus are:: genital warts, fissure and fistula, hemorrhoids, marginal thrombus, cancer and itching. A basic rectal examination involves viewing, "right" (finger penetration) i anoskopię (wziernikowanie).

  • Ostuda

przebarwieniaOstudy to duże, dobrze odgraniczone plamy nieregularnych kształtów, występujące najczęściej na skórze twarzy kobiet, zwykle symetrycznie. Powstają w ciąży, w wyniku zaburzeń miesiączkowania, stanów zapalnych jajników, zaburzeń endokrynologicznych, chorób wątroby, tarczycy, wyniszczenia, stosowania niektórych leków i kosmetyków światłouczulających, leków hormonalnych.

  • Piegi

przebarwieniaPiegi są małymi, brunatnymi plamkami umiejscowionymi na skórze narażonej na działanie promieni słonecznych, mainly on the face. They most often occur in people with pale skin and light or red hair. They are hereditary in nature. They can be safely removed with exfoliants, microdermabrasion, laser, but usually the effect is only temporary. Their formation can be prevented by avoiding exposure of the exposed body to sunlight or by using creams with strong UV filters.

  • Antithrombotic prophylaxis

antithrombotic prophylaxisWhy do we use antithrombotic prophylaxis??

We prevent because prevention is cheaper than cure. To avoid complications of deep vein thrombosis (DVT) – pulmonary emboli, post-thrombotic syndrome, leg ulcers. Obecnie najczęstszą formą farmakologicznej profilaktyki przeciwzakrzepowej jest stosowanie iniekcji podskórnych z heparyn drobnocząsteczkowych (preparaty Clexane, Fraxiparine, Fragmin).

    Wskazania medyczne do stosowania profilaktyki

    Przeciwwskazania do leczenia przeciwkrzepliwego:

    • skaza krwotoczna
    • objawy krwawienia z przewodu pokarmowego
  • Prosaki prącia i warg sromowych

pearly papulesNazywamy tak niewielkie, położone śródskórnie torbiele wypełnione masami łojowo-rogowymi, resulting from retention of the secretion of the sebaceous glands due to excessive keratinization of the hair follicle openings. They do not cause any discomfort, They are usually up to 2 mm in diameter and white or yellow in color, and comparing their appearance to millet grains gave rise to the name "millets". In foreign nomenclature they are known as Fordyce spots or Fordyce's disease. They cannot be removed pharmacologically or by puncturing or squeezing, and such behavior only causes inflammation and skin abscesses. Basically milia on the penis, scrotum or vulva do not require treatment and are removed for aesthetic reasons.

Rozróżniamy przepuklinę prostą i skośną.

niewydolność żylna, varicose veinsDefinicja i epidemiologia

    • układ żył powierzchownych; obejmuje on żyłę odpiszczelową, odstrzałkową i ich bocznice.
    • układ żył głębokich; żyły te przebiegają podpowięziowo i towarzyszą tętnicom.

„niespokojnych nóg”, rozpieranie i pobolewanie stóp i podudzi, nocne kurcze łydek. Niewydolność zastawek, skutkująca podniesieniem ciśnienia hydrostatycznego w układzie żylnym kończyn doprowadza do poszerzenia pni żylnych, ich wydłużenia i poskręcania (rozwoju żylaków). W bardziej zaawansowanym stadium choroby dochodzi do stwardnienia i zaniku tkanki podskórnej i stanów zapalnych skóry.

    Diagnostics and differential diagnosis

The basic phlebological examination involves a carefully collected medical history and physical assessment of the patient. Band tests are helpful in assessing pathology, limb circumference measurement and Doppler examination, however, not always sufficient. Currently, the "gold standard" in the diagnosis of vein diseases, especially the deep system, jest ultrasonography, which allows for imaging morphological changes. One of the tests that is less and less frequently used in phlebological practice is photoplethysmography, phlebography, capillaroscopy, mikrolimfografii, Isotope studies of thrombus, computed tomography, magnetic resonance imaging or thermography. The differential diagnosis should include damage to the musculoskeletal system, torbiel Beckera (pęknięcie torbieli), obrzęk chłonny, różę, cellulitis, choroby internistyczne przebiegające z obrzękami obwodowymi.

    Leczenie i profilaktyka

Ogólnie możemy wyróżnić leczenie inwazyjne i zachowawcze. These include:. leki flebotropowe, heparyny i heparynoidy, doustne antykoagulanty, fibrynolityki, leki sklerotyzujące, niesteroidowe leki p/zapalne, diuretyki, leki p/płytkowe, pochodne ergotaminy. Doustne leki stosowane w przewlekłej niewydolności żylnej mają na celu zwalczanie takich objawów jak obrzęk i uczucie ciężkości kończyn dolnych, objaw niespokojnych nóg, kurcze nocne. Działanie leków flebotropowych polega na poprawie funkcji mikrokrążenia (uszczelnienie bariery włośniczkowej), zmniejszeniu przepuszczalności naczyń żylnych, zwiększeniu tonusu (m.in. na skutek przedłużenia skurczu włókien mięśniowych ściany naczyń żylnych w odpowiedzi na noradrenalinę), poprawie drenażu limfatycznego, właściwościach antyagregacyjnych, poprawie właściwości reologicznych krwi, zmniejszeniu jej lepkości oraz zmniejszeniu reakcji zapalnej towarzyszącej zastojowi żylnemu (hamowanie wydzielania mediatorów zapalnych). Leki te można stosować w monoterapii lub łączyć z innymi metodami postępowania. Wczesne ich włączenie i stałe przyjmowanie znacznie spowolni postęp choroby i opóźni wystąpienie ewentualnych następstw. Stosowane leki możemy podzielić na naturalne i syntetyczne. Among commonly used drugs, the largest group are preparations containing natural and synthetic flavonoids, mainly rutoside, diosmin and hesperidin, and also rutoside derivatives – β-hydroxyethylrutosides. A well-known and proven drug is diosmin (a semi-synthetically produced analogue of a natural flavonoid isolated from citrus fruits), available in several preparations. High therapeutic effectiveness, good tolerance and high patient satisfaction combined with high safety of use (no contraindications except allergic ones, no interactions with other drugs), convenient dosage and favorable price ratio determined the high market position of this drug. The properties of phlebotropic drugs described above in the case of diosmin result in, among others:. faster disappearance of symptoms, faster resolution of symptoms in patients with venous thrombosis, accelerating the healing of leg ulcers. This is reflected in numerous scientific reports in the field of phlebology. We also use compressotherapy to treat chronic venous insufficiency (compression treatment). It increases the speed of blood flow in the veins, reduces the diameter of the veins, improves the function of venous valves, reduces swelling. This is a very effective treatment and should be used at every stage of the disease. Bandages are used for this purpose, opasek elastycznych lub specjalistycznych podkolanówek, pończoch i rajstop o stopniowanej kompresji. Fizykoterapia to głównie leczenie ruchem i stosowanie odpowiedniej aktywności fizycznej.

Wymaga znieczulenia pacjenta.

  • Septyna

The test is intended for people at general or moderate risk of developing colorectal cancer..

  • Anal fissure

anal fissureAn anal fissure is an ulceration of the final part of the anal canal, resulting from a longitudinal crack in the skin covering it (anodermy). The most common cause of its occurrence is mechanical trauma (constipation, diarrhea), often with increased tension of the anal sphincters. In most cases, it occurs on the back wall of the canal. It manifests itself with burning pain, itching and slight bleeding after passing stools (defecation).

It may appear unexpectedly and go away on its own. For this reason, treatment is difficult and relapses are frequent. Persistently persistent ailments lead to irritation and irritation, causing conflicts with the environment, lower professional activity and quality of life.

In any case, by collecting a medical history, examining the patient and performing additional tests, We are trying to determine the etiological factor of the condition.

These may be the so-called. systemic diseases such as diabetes, gout, inflammatory diseases of the kidneys, hyperthyroidism, allergies, liver failure, mechanical jaundice, focal infections, digestive disorders, iron deficiency, ovarian failure, morphinism, nieswoiste zapalenia jelit lub przyczyny miejscowe: niedostateczna higiena osobista, drażniące wydaliny lub wydzieliny z sąsiednich narządów, podrażnienie chemiczne lub mechaniczne, zakażenia grzybicze, pasożyty, stany zapalne skóry, zmiany łuszczycowe, venereal diseases – szczególnie kłykciny kończyste, wypadanie hemoroidów lub odbytnicy, anal fissure, przetoki okołoodbytnicze, cancer.

It is necessary to remove the lesion in its entirety, including the thin surrounding capsule. If the treatment is not radical, the lipoma grows back.

  • Pilonidal cyst

pilonidal cystPilonidal cyst (pilonidal cyst, pilonidal sinus) it arises from inflamed hair follicles and contains hair, that grow inside her. The etiological cause of inflammation is most often anaerobes. The process takes place in the subcutaneous tissue in the sacral area (buttock gap). This then leads to the formation of cysts with purulent fistulas in the skin. Fistulas may become temporarily closed, accompanied by exacerbation of disease symptoms. An abscess then forms, accompanied by typical symptoms (fever, pain, swelling and redness in the sacrum area).

  • Waporyzacja

Są to: wędzidełka warg – górnej i dolnej, łączące je z dziąsłami, wędzidełko języka, łączące go z dnem jamy ustnej i wędzidełko napletka. Wędzidełko napletka to silnie unerwiony cienki fałd łącznotkankowy, łączący żołądź prącia z napletkiem.

Skrócenie wędzidełka wargi dolnej, wskutek odciągania dziąsła od zębów, powoduje anemizację błony śluzowej dziąsła przyczepionego do wędzidełka i odsłanianie szyjek zębowych.

Ma wygląd lekko wypukłego, twardego guzka o zabarwieniu sinoczerwonym do brązowego, o nieostrych brzegach, wielkości do 5 mm i obejmuje całą grubość skóry. Statystycznie najczęściej jest spotykany na podudziach. Nierzadko powoduje świąd, co skłania pacjentów do wizyty u lekarza. Jedynym leczeniem jest chirurgiczne wycięcie zmiany bez zbytniego marginesu zdrowej skóry. Wymaga różnicowania z czerniakiem. W razie wątpliwości diagnostycznych rozpoznanie musi być poparte badaniem histopatologicznym preparatu.

  • Włókniaki miękkie

wyrośla skórneSą to rozrostowe zmiany tkanki łącznej, występujące najczęściej w formie mnogiej.

Odbarczenie przynosi szybką ulgę, a powrót do zdrowia zajmuje kilka dni. Nieoperowany zakrzep ustępuje samoistnie, lecz gojenie jest dłuższe. Mogą też wystąpić powikłania w postaci martwicy skóry i perforacji zakrzepu.

Zmiana szybko rośnie, wykazując tendencję do kontaktowego krwawienia.

    Fakty i mity:

      • znamiona „podejrzane” leczymy wyłącznie chirurgicznie i badamy histologicznie

We should also remove moles that are subject to constant irritation (shirt collar, trouser belt, bra strap etc.)

  • Sutton's birthmark

moleSutton's birthmark ("halo" birthmark) it is a special type of mole, around which there is a rim of white skin, causing the patient's concern and a visit to the doctor. A pigmented nevus is most often several mm in size, it is of regular shape with a clear border and various colors, with border to 1 cm of discolored skin. It occurs mainly in children and adolescents 20 years of age, most often on the torso. Phenomenon “halo” it is caused by an unknown cause of cytotoxic reaction of T lymphocytes, która skierowana jest przeciwko komórkom barwnikowym obecnym w samym znamieniu oraz w otaczającej go zdrowej skórze. W wyniku niszczenia melanocytów dochodzi do zmiany zabarwienia znamienia, początkowo w części środkowej, następnie całe znamię może ulec zanikowi, a otoczka ulega wtórnej repigmentacji.

W przypadku wszelkich wątpliwości i znamion budzących niepokój lekarza należy chirurgicznie usunąć zmianę i poddać ją ocenie histopatologicznej.

    Warto wiedzieć:

    – żylna choroba zakrzepowo-zatorowa (ŻChZZ) występuje bardzo często zarówno u chorych hospitalizowanych, jak i w warunkach pozaszpitalnych i w większości przypadków ma klinicznie niemy przebieg;
    – większość zakrzepic okołooperacyjnych przebiega bezobjawowo. Istnieją badania stwierdzające, that u 40% pacjentów operowanych bez profilaktyki przeciwkrzepliwej występują zmiany zakrzepowe kończyn dolnych;
    – pierwszą manifestacją choroby może być zgon spowodowany ZTP, dlatego nie należy czekać, aż wystąpią objawy ZŻG i dopiero wówczas przeprowadzać postępowanie diagnostyczne i wdrażać leczenie;
    – nierozpoznana i nieleczona ZŻG może mieć odległe następstwa w postaci zespołu pozakrzepowego, usposabia chorych do nawrotów ŻChZZ, a w części przypadków jest przyczyną przewlekłego nadciśnienia płucnego;
    – leczenie powikłań ZŻG jest przewlekłe i kosztowne;
    – czynnikami inicjującymi zakrzepicę żylną są 3 składowe – tzw. triada Virchowa:

    • uszkodzenie ściany naczynia
    • wzmożona krzepliwość krwi
    • zwolnienie przepływu krwi.

    Czynniki ryzyka ŻChZZ:

    • wiek >40 lat
    • długotrwałe unieruchomienie
    • udar mózgu z porażeniem lub znacznym niedowładem kończyn
    • przebyta ŻChZZ
    • nowotwory złośliwe i leczenie przeciwnowotworowe
    • duże zabiegi operacyjne (szczególnie w obrębie kończyn dolnych, miednicy i jamy brzusznej)
    • urazy (zwłaszcza złamania miednicy, bliższego odcinka kości udowej i innych kości długich kończyn dolnych)
    • obesity
    • żylaki kończyn dolnych
    • niewydolność serca III i IV klasy NYHA
    • obecność cewnika w dużych żyłach
    • choroba Leśniowskiego-Crohna, wrzodziejące zapalenie jelita grubego
    • zespół nerczycowy
    • czerwienica prawdziwa, nadpłytkowość samoistna i nocna napadowa hemoglobinuria
    • ciąża i połóg, stosowanie doustnych środków antykoncepcyjnych i hormonalnej terapii zastępczej
    • trombofilia wrodzona powodowana niedoborem naturalnego inhibitora krzepnięcia lub mutacją genów
    • obecność przeciwciał antyfosfolipidowych

    • obrzęk kończyny (konieczny pomiar obwodów obu kończyn i stwierdzenie asymetrii)
    • Homans' sign - severe pain in the lower leg when dorsiflexing the foot
    • excessive filling of superficial veins
    • change in skin color to bluish-red
    • Mosel sign – increased tension and pain in the affected area.

Diseases and conditions with similar clinical symptoms:

    • damage to the musculoskeletal apparatus
    • zespół pozakrzepowy
    • cellulitis
    • superficial thrombosis (superficial thrombophlebitis)
    • rupture of Baker's popliteal cyst
    • obrzęk chłonny
    • stasis (reflux) varicose veins
    • rose
    • some diseases of internal organs.
